Purpose of Position:

To provide mental health therapy and related services to high risk families, referred by Department of Human Services, whose children are at substantial risk of out-of-home placement or who are in placement and cannot be reunited without in-home services. Typical issues to be addressed in therapy may be child maltreatment and positive parenting skills, domestic violence and abuse trauma, depression and substance abuse, and other significant mental health issues that contribute to family dysfunction.

**Summary of Essential Job Functions: **

Direct Services (50%)

Provide in-home therapeutic services and support to families referred by DHS Child Welfare and identified as meeting the criteria for Differential Response as established by NDI and DHS Child Welfare. In-home mental health services will last approximately 1 hour. Initially services may be 2 times a week in order to assist the family in making a safe environment for children. Maximum case load will be 15 with most families being seen weekly or every other week.

• Provide a Mental Status Examination on each identified family member;

• Establish an Family Services and Support Plan (FSSP) based on a current assessment and information provided by the referring agency for the family members;

• Provide mental health and family services as indicated by the FSSP. Services may include, but are not limited to individual and/or family therapy, skill building, case management, and other mental health services;

• Provide crisis counseling, interventions and support as needed, suicide evaluation, grief counseling etc.

Travel (25%)

The In-home Therapist is responsible for providing therapeutic services to families in Hood River and Wasco Counties which may cover hundreds of miles. Scheduling and coordinating services to best facilitate family schedules is essential, as well as time management for travel.

Documentation (15%)

• Complete written Family Support and Services Plan (FSSSP);

• Maintain chart notes on each individual or family session using concurrent documentation ;

• Provide monthly progress summaries to DHS and stakeholders;

• Keep client files current and up to date as required;

• Consult with other professionals concerning identified clients or professionals directly involved in providing services for the clients.

Collaboration (10%)

• Coordinate with other service providers, schools and involved individuals and agencies;

• Participate with the identified treatment team in development, implementation and delivery of treatment according to the Master Service Plan;

• Observe confidentiality, civil rights and other rights as established by the therapy agreement;

• Make referrals for additional services and or work with clients to obtain necessary services;

• Encourage positive community relations by personal appearance and behavior in the community;

• Special projects as assigned by Treatment Services Manager or Treatment Services Director. Competencies and Skills Requirements:

• Good technical skills and competent use of computer programs and software Education and Experience Qualifications:

• Accredited Master’s Degree in Human Services or related field

• Experience in mental health and family therapy including assessment, treatment planning and service delivery

• Must meet credentialing requirements as a Qualified Mental Health Professional
